Resource Text for Hypertension and the New GMS Contract II

Domains
QOF has been updated in April 2006 with new indicators added and in some areas there has been new point allocation. The total number of QOF points is unchanged and the money per point has also remained the same as last year. However, there have been nine new indicators introduced which total 138 points. The QOF review process started in February 2005 with the appointment of the University of Birmingham in conjunction with the Royal College of General Practitioners (RCGP) and the Society of Academic Primary Care to form the expert panel. An open call for evidence was then held from 18 April 2005 to 30 May 2005. In total 514 submissions were received. The expert panel reviewed all the submissions and provided recommendations for both modifications of existing indicators and potential new indicators to the negotiating parties, National Health Service (NHS) Employers and the General Practitioners Committee (GPC).

The QOF system still comprises four domains for evaluation. Two are mainly clinical and two are more non-clinical. Each domain contains a range of areas described by key indicators. The indicators describe different aspects of performance.

The four domains are:

I. Clinical domain
This covers ten disease areas:

coronary heart disease (CHD) including left ventricular dysfunction (LVD)
heart failure
stroke and transient ischaemic attacks (TIA)
hypertension
hypothyroidism
diabetes
mental health
chronic obstructive pulmonary disease (COPD)
asthma
epilepsy
palliative care (new indicator)
dementia (new indicator)
chronic kidney disease (CKD) (new indicator)
atrial fibrillation (AF) (new indicator)
obesity (new indicator)
learning difficulties (new indicator)
cancer.

II. Organisational domain
This covers five areas:

records and information
communicating with patients
education and training
medicines management
clinical and practice management.

III. Additional services domain
This covers four areas:

cervical screening
child health surveillance (CHS)
maternity services (MAT)
contraceptive services (CON).

III. Patient experience domain
This covers two areas:

patient survey
consultation length.
